(USES) is pleased that you have decided to work with us in our response efforts. To make our invoicing process more effective, USES has included the following list of provisions for you to follow when invoicing USES for work on this project. All Subcontractors will need to invoice USES per the approved rate schedule. This schedule is project specific and is subject to change if new rates are negotiated with the client. Subcontractor Per Diem for this project will be invoiced to USES at $85.00 per day per man. Subcontractor personnel will be required to sign daily sign-in sheets per USES protocols upon beginning and ending work each day. If the sheets are not properly signed, the Subcontractor will not be allowed to invoice for the employee in question. All Subcontractors will need to submit their completed daily field tickets (through midnight of that day) to the USES Point of Contact by 0700 the following morning. All Subcontractors will need to provide the following documentation for each employee: A copy of a Photo Identification that clearly states the name of the employee. The Subcontractor must invoice USES utilizing this name. A copy of a 40-hour HAZWOPER certificate for each employee, and An 8-hour refresher certificate, if applicable These items will need to be uploaded here and approved before the employee can begin work on this project. These are the steps to register: Call USES and request a contractor administration account. Create employee records for each employee. Upload scans of certificates, photo ID and I-9 for each employee. USES staff will review your materials and approve your employees or inform you what is missing. After employees are approved you may begin entering dailies. Use of this site indicates your agreement to the Contractor Agreement. We may be reached 24-hours every day by calling 504-279-9930. Our goal at USES is to ensure an excellent customer experience from the moment our customers call for service to the time we bill and collect for the services provided whether it be planned or emergency response. The USES Operations Portal is our mechanism to ensure that all requests for either environmental or industrial services are processed efficiently; the Customer is billed correctly based upon their contracted rates; and provide ourselves and the customer with centralized and reliable information to measure and report the quality of our performance. Governing Policy All project or service related field tickets must be submitted to the local USES Division office on a daily basis and then entered into the Operations Portal. Data entry and billing for all work performed during the week must be completed by Friday of the following week. This includes: • All labor, equipment, materials or subcontract/cost plus items for completed or in process service tickets performed during the week • Billing data entries must be entered based on a signed customer work order. Objectives The objective of the Service Billing Process is to: • Ensure consistent and complete entry of weekly employee time; • Bill customers by the end of the week following work completion; and • Input all billable and non-billable time, equipment, material and subcontracted work on a regular basis.
